A new slayer is only called when the current slayer is killed. Therefore, Buffy had already died at this point and a replacement slayer had been called for her previously.
A new slayer was not called because when Buffy died The 1st time Kendra was called and then when she died Faith was called so when Buffy died again Faith was still around so a new slayer could not be called until both slayers (Buffy an Faith) are dead.
Usually, when a slayer dies, the next one is activated. In Buffy's case, this was Kendra. Kendra was killed, hence, Faith was activated. The activation of the next slayer only happens once in Buffy's case. Technically, Buffy had already died once before 'The Gift'. Her replacement had already been activated and killed, so there wouldn't be another when she died the second time. If you want to get REALLY technical, Buffy died 3 times on the show. Although it was an alternate dimension, she was killed in 'The Wish'. So REALLY technically, there is an alternate dimension Kendra, presumably still alive, thus no Faith.
Yes, in season 5 (not sure on the exact episode). Buffy walks through the door announcing she is home only to find Joyce dead on the couch. She died from a brain tumour. This is backed in season 7 "Conversations With Dead People" where either her spirit or The First in her form and speaks with Dawn.
SPOILER ALERT Buffy, Faith, Kendra and all the girls with the potential to be a slayer, so (in the comics) over 1,200 girls, some of which include Kennedy, Amanda, Rona, and Vi, although Amanda dies.
Yes, Buffy dies twice in Buffy the Vampire Slayer. She dies first in season one when the master drinks from her and she drowns, but Xander manages to bring her back. Then she dies once more in the season five finale when she sacrifices herself for the benefit of the world and for her little sister, the original sacrifice. At the beginning of season six, Willow finds a special way to bring her back again but it has a great price.

No, neither does Buffy. Drusilla never died
The First can take form of any living thing if it has died, not if it is dead. Buffy has died twice, so... Yeah.

The line goes as (buffy)- (kandra)- faith then in the last episode willow makes all the potential slayer full blown slayers to stop The First (parenthisies mean dead or has died)
