Two immediate similarities in my mind are that they both possess a bureaucracy to implement their rule and maintain police and/or military forces to control force.

In totalitarianism the people and government are controlled by a central body or dictator (tyrant etc) without input from the public. The antonym would be a direct democracy where the people vote on issues directly without representatives in parliament or senate (representative democracy). Chaotic forms of government such as Anarchism (no rules at all) would be another potential acronym.
