Saw IV is about how a police officer is put through a round of tests while the FBI investigates the Jigsaw killings. If you're thinking about watching it, I highly recommend it. It was very well done, and if the gore from the third movie left you not so excited to see this one, don't worry. It's not as bad. This movie also has an extremely remarkable tie-in to the 3rd one and others which you never see coming.

The reason why Saw didn't have as much gore was because the producers didn't have enough money to make it as bloody as they wanted, because the studio didn't want to pour money into a movie that potentially could flop and not make very much. However, once they saw (get it?) that the franchise was marketable, they were willing to put more money into it, and they could make more gore.
