Being an epic spaghetti western one assumes that it was shot in Italy, rather it was shot in Spain (with Franco's fascist Army as extras) by Italians, with an American cowboy TV star but made to look Tex-Mex.
If you love the longing stares and stylized violence of this movie remember Leone wanted to satirize Westerns for their exaggerated violence. Leone stole a lot of his ideas from a famous Japanese director who lived through the the 1940's destruction of Japan (and who eventually sued Leone in court). Educated young people of the time read them as tongue-in-cheek attacks on violence and war. It just so happens that young Americans and Australians where very anti-war at the time because it seems as teenagers they were old enough to be forced to fight and die (and worse) in war but not old enough to vote for their own leader, so one of the reasons it enjoyed cult status was because it was often read as an anti-war movie.

Because the Good removed the bullets from the Ugly's gun without the Ugly knowing about it. The Good (Clint Eastwood), the Bad (Lee Van Cleef), and the Ugly (Eli Wallach) face off in a cemetery at the end of the movie. The Good kills the Bad as the Ugly realizes his gun isn't loaded. Then the Good forces the Ugly to stand atop a rickety wooden cross with a noose around his neck. As the Good rides away he shoots the noose, freeing the Ugly. The Ugly then realizes that the Good has left at least one bag of gold. So, although he lost the duel, at least the Ugly makes off with some reward for his troubles.
