"Saving Private Ryan" is a very gruesome war movie that I have seen. "Brave Heart", however, is considered to be very gruesome as well.
An Honorable Mention would have to go to the Russian version of "War and Peace". It was 7 hours long and there where scenes of massive troops in battle and it was quite realistic.
"300" is an animation movie that is quite bloody.
