Yes he does, in manga chapter 348, killed by Ulquiorra, but Inoue shouts out his name for help on what she should do causing him to come back to life in his hollow form and almost kills Ulquiorra when Ulquiorra attacks from behind, causing Ichigo to go back to his origanal state, not remembering what had just happened.

Eventually i believe he does and when i find out the episode i will tell you although Ulquorra almost killed Ichigo once thnk god for Orihime... well actually Grimmjow. He told her to save him so that he could fight and kill him but he ends up losing and dies <Tears> Bye bye Grimmjow I'll just say yes Ichigo does defeat him at great costs though, see ichigo was basically Killed by Ulquiorra (Again) using his weird finger light attack. ichigo was then urged back to life to protect Orihime who was screaming his name he then chants to himself and his hollow decides to basically turn him into something... extremely dangerous not exactly a visord.. more arrancar-like. ichigo is not conscious during the time that the hollow destroys ulquiorra ruthlessly. To answer your question, Ulquiorra's sword/release word is Murcielago (or Murushierago if you read it in the manga since they spell it with katakana so either works fine). Of course you probably already know the answer by the time I answer this haha. Even Better in Japanese it is ムルシエラゴ
