Well, any names are perfectly fine for a scary story unless you are setting the story in a specific time period or area that would use particular names. You wouldn't find an Indian named Bob Smith during the 1600s! Sometimes picking names purely for "scare" purposes can backfire. Having every character have a goth name or supposedly scary name can make the story seem amateurish.
Carnivals, amusement parks, hospitals, a family home, etc. are often used for scary movies.

Well, it depends on what your story is about. Vampires, Werewolfs, Monsters, ghosts etc. Titles can be inspired by so many different things. Plus, it is your story so maybe you should be the one to choose. Good luck on finding one..
Jefferson Jr high school is haunted my teacher told us alot of stories when he was first teaching there trust me its scary one of the stories were that a dude name bob was working on the concrete and he had fell in and the machine was still running and he got buried in the cement. that only part of the story i still hear voices there trust me its scary there a whole different story..
