If you think there is something inaccurate in your records then speak with the Doctor Who put them in or a specialist in the field. Have them amend the record with a current status record. If the doctor will not do that or you still disagree then there is little you can do. Those who would look at your records like insurance companies are not likely to take your word over a doctors.

If you are in Canada under the Personal Health Information Act patients have the right to instruct the custodian of the information that they want all or part of their record to not be disclosed to anyone. It is referred to as a lock box provision. You can also place a addendum on your file disputing what the doctor has written but as we all know physicians will pay no attention to your information. This lock down provision can be at both hospitals, clinics as well as making your family practioner the only person to have access to your records in his office.
