Well, first you get the sheet music, then you transpose (or arrange) it for Clarinet if that's not already been done, then you play it. I'm not sure what you were really wanting from us, since I'm not about to sit here and type out all the notes, even if I knew them. (Among other reasons, it's probably a violation of copyright.)
If you're asking where you can get the sheet music, you might try musicnotes.com, or a local music store. It shouldn't be too hard to extract the melodic line from the piano score.

the clarinet cant play chords, it impossible unless theres a quartet and each plays a separate note. however it can play all broken chords and appreggios :P

The reed is the main part of the clarinet. Without it you cant play the instrument. It allows you to blow threw the clarinet to make the noise. This goes along with some other woodwind instruments (the flute and recorder are the only woodwinds that I can think of that don't need a reed).

To answer your question on what is a clarinet. It is a woodwind. You do not know what a woodwind is well I know. There is different woodwinds there is a clarinet, flute,saxaphone, and oboes. Nobody knows where the clarinet got it's name. I just play the clarinet.
