No one really "designed" heavy metal. It all came about with certain bands/artists writing certain songs and using certain styles. A music type has to develope over time. However, Alice Cooper did created his own genre of music called "shock rock" because of all the songs he created. That is an acception to the rule. However, Alice Cooper did influence many bands that are now categorized as heavy metal today (such as KISS, Marilynn Manson, etc).

Alter Metal is a cross genre between Grunge/ Alternative and Heavy Metal. Anter Metal and Alternative Metal are different in many ways, such as Alter Metal bands have a grungy look to them. And Alter Metal leans more towards Grunge/ Alternative, and Alternative Metal is more Heavy Metal.
Interesting as there very well could be a music form which is essentially heavy metal influenced by country or "western" music. As for bands, there aren't that many, but Dezperadoz (from Germany) is a metal band that has western influences.

He has changed the world mainly because of his work with Black Sabbath, because they were the pioneers in Heavy Metal, and influenced many bands. Actually, many of these bands wouldn't even be there without Sabbath!

Almost none. It depends. Just google it. But if you hear it on mainstream American radio, it's not death metal.
Many Heavy Metal bands wore platforms, in the 70s, and early 80s. Nowdays some bands still wear them as a gimmick. This was known as 'GLAM ROCK'. The band that springs to my mind is KISS. This was by no means the first band to do so. DAVID BOWIE was also a big wearer of platforms. ha ha boom boom
