It is because when Rabindranath Tagore wrote it, the whole of India was not united.

The names of the places are Punjab, Gujarat, Maratha(Maharashtra), Dravida(South India), Utkala(Orissa), Banga(Bengal), Vindhas, Himachal. The names of the rivers Sindhu, Yamuna and Ganga are also mentioned, denoting the places they cross. But other important places like Uttar-Purvanchal(North-East India) etc. are not mentioned.
