There can be many causes of a rough idle. But if the engine runs good under acceleration and then idles rough, there are 2 things I would look at. First look to see if you have a vacuum hose disconnected, broken, or cracked. Secondly depending on the make and model, check the EGR valve. This is just the main 2 causes as there are more.
-also rotor and distributor cap arcing - use only original dealer parts
-O2 sensor slow - if it is old replace
-ignition wires old or incorrectly routed
-if gas smell do a fuel pressure leak-down test - should hold 50psi for 10 mins or more. May be pressure regulator or line leaking or bad fuel pump

I own a 98 Lexus ES 300. I struggled with the same issue. The problem was the EGR Valve. It is an emission part. The total part and installation was around $300.00. It was a little expensive, but it cured the problem and have not had any problems since then. Hope this helps. Mark
