Nativity In Black. It's not Nativity in Black, the name of the track was just a nickname (Geezer Butler thought Bill Ward's beard looked like a nib). There is no real meaning, they are singing from the point of view of the Devil. Perhaps saying that even evil has a heart? Personally I think they just thought it sounded good.
