The name of the song is officially "Judy in Disguise (With Glasses)". The original recording was by John Fred and his Playboy Band; two members of the band (Fred himself and Andrew Bernard) wrote it. Reportedly, the idea was based on The Beatles song "Lucy in the Sky With Diamonds" which Fred misheard as "Lucy in Disguise With Diamonds".
By the way, the lyrics are truly bizarre. I'm guessing Fred figured that if Lucy in the Sky with Diamonds and I am the Walrus could be hits, "bizarre" was exactly what those crazy teenage kids wanted. And the melody is extremely catchy... catchy enough that it wound up knocking another Beatles song out of the number one spot.
