The story was originally a novel by a Frenchman, Gaston Leroux, who published "Le Fantome de l'Opera" in 1911. In English we'd say "The Opera Ghost" or "The Ghost of the Opera", but while the notes from the Phantom in the translation are signed "O.G.", he's still largely referred to as "the Phantom", and the rather more romantic/gothic "The Phantom of the Opera" was used as the title when the book was first translated into English. "The Ghost of the Opera" (let alone "The Opera Ghost") just doesn't have the same ring to it, and I suppose English readers who had heard of "Le Fantome de l'Opera" may not have recognized the book had "fantome" been translated as "ghost" rather than "phantom". But in French it's the same thing.

Ever since then, he's been referred to as "the Phantom", although a lot of versions will also refer to the "ghost" that apparently haunts the opera, especially if the scene has backstage workers talking. I guess they use the simpler "ghost" rather than "phantom" because they're usually depicted as being rather lower-class. The translation of the novel uses both "Phantom" and "Ghost", but in the original French they just use the one word.

The Andrew Lloyd Webber musical doesn't actually give him a name, preferring to maintain an additional layer of mystique by simply using "Phantom" throughout; but in versions where he *is* named, he's "Erik" after the novel (in which his last name is never given). Only a very few people ever know his actual name, though.

I suppose the short version is that as most of the characters only know of him as this ghost that haunts the opera, that's how they refer to him, with "phantom" usually being used rather than "ghost" as it's closer to the word used in the original French.
