We could just as easily say "improve symptoms" as "reduce mortality", its all about what the new medicine is expected to do. In statistical terms, there's a different approach stated as a null hypothesis, which is not a question but a simple statement that there is no difference between the new and current drugs, something like "There is no difference in mortality between new drug and old drug." We then perform statistical tests to see if a difference is present - if we can't find a difference, we conclude the drug is truly no different.
