A dentist.
If you have a hole in the mouth with a white line around where a tooth use to be mean that you have a tooth coming back in. I would go to the doctor and have them check it out.
The specialist in the area of soft tissue inside the tooth is an endodontist. Endodontists specialize in the diagnosis and treatment of issues related to the dental pulp and tissues inside the tooth, such as root canals.
Nothing can escape from a black hole.
The small pin hole in the first molar is the natural opening of the tooth's root canal, called the apex. This opening allows for nerves and blood vessels to enter the tooth to supply nutrients. It is a normal part of tooth anatomy and does not indicate any problem.

"Cary" (one r ) is the real, or, medical, term for a hole in a tooth due to tooth decay. These are commonly referred to as "cavities" however a "cavity" technically can be a hole or depression in anything. A Cary is specifically a hole in a tooth.
The bump is an abscess, the inside of your tooth is rotting and full of bacteria. These bacteria come out of the root of the tooth where the blood supply and nerves went into the pulp of the tooth when it was healthy. When they reach the jawbone the body can detect them (it can't so well when they are in the tooth) and begins to fight them causing pain and swelling and the bump is full of puss. The root canal work cleans out the inside of the tooth and drains the puss through the hole in the tooth root. Left untreated the abscess will continue to grow (fed by infection from inside the tooth), destroying jaw bone until it bursts though to the surface of the gum.
A hole is a cavity, it means that the tooth is rotten or rotted. If you see a hole it is past time to see the dentist. You should also be brushing more often.
