A "FELLOWSHIP" is the term for a period of time (usually one or two years) where a person whom is already a "general" practitioner (already a "doctor" in general medicine who can diagnose, treat, and prescribe medication independently in general medicine) becomes a "fellow" in a desired field of medicine (such as obstetrics or oncology) and thus has a supervising physician whom is already a specialist in that field that they work under until their training is completed.

Once their training is completed, they take the testing involved in becoming a "Board Certified" specialist in that area of medicine. (this answer is specific to the United States and Canada)
