I found a reference to why the street was named Polhemus - In February, 1865, C. B. Polhemus was a director of the new San Francisco to San Jose railroad. My guess was that the street was named in honor of him given the street's close proximity to the College Park flag station. As for the name change, it was done in the 1960's and where Naglee hits Alameda the city had to do a funny curve where Naglee becomes Polhemus/Taylor. My guess is that the Taylor name won out.
