Part of the meaning of Midsummer Night's Dream is that love is irrational. We know that Demetrius falls in love with Helena because of a magical love-juice but there is no rational coherent reason why Helena should love him either. She just does.
Demetrius does not love Helena. Helena loves Demetrius. But he loves Hermia.
All this changes later in the play when Demetrius is drugged into loving Helena instead by the fairy Puck.
