A Deputy Stage Manager (DSM) is in control of the book. In rehearsals they must write down all the blocking for each scene as they are blocked and make changes as appropriate. They must also take rehearsal notes and at the end of the day send them out to each department so anything that needs attention can be sorted.
In smaller companies, where there is only one member of the stage management team, the stage manager will attend all rehearsals to create the prompt book. The prompt book contains all blocking and cues as well as all the relevant paperwork for a production. On larger shows, this role is often given to the deputy stage manager. The stage manager will also liaise with the director about all aspect of the show, especially props.
In modern theatre, the stage manager is in charge of rehearsals, under the direction of the director. In Shakespeare's Midsummer Night's Dream, Peter Quince appears to be both stage manager and director. And he is also the author of the play! This kind of multitasking in offstage roles is pretty common in amateur theatre.

The three members of the Stage Management team are: the Stage Manager (SM), the Deputy Stage Manager (DSM) and the Assistant Stage Manager(s) (ASM). A large scale West End show will have a SM, DSM and several ASM'S whereas a small touring show is likely to only have an SM.
