You can use Blender, and then get the PMD plugin to convert between blender and pmd formats. This is useful for bone rigging for PMD, because it's hard to rig bones in PMD editor in comparison to blender. I'm planning on making models this way. There's also metasequoia, but the free version is quite limited, and I have no idea where to convert it directly to pmd file because you have to convert it .x file to upload into PMD editor and make it .pmd.

You have to get PMD Editor which is an MMD accessory. Search Google for "VPVP Wiki", you should be able to get it there.

make sure they are the latest versions of things and if it is, delete and remove pmd editor, download it and start it up. if still doesnt work, make sure your video card is compatible with direct x 3D

The best way ( if u want to make them from scratch) is to use Blender and make the body , eyes, clothes , hair ect.... or u can edit models by using PMD editor ~ hope this helped ~
