The real king Macbeth's wife's name was Gruoch (you can see why Shakespeare didn't use it). It was her second marriage; by her first to Gillecomlan she had a son, Lulach, who succeeded Macbeth as king.

Shakespeare never stated her first name. Back in his time, women were only addressed with their husband's last name.

Once she married Macbeth, she was and forever would be Lady Macbeth

If I might add to the above answer (which I believe to be totally correct), it should be pointed out that in William Shkespeare's play, and also in historical accounts of the actual person, Mr. Macbeth had no first name also...but at least Lady Macbeth had a title!

Macbeth's first name was Macbeth. That's the Anglicized version of his name. His name in Gaelic is MacBethad. In modern Gaelic, it's MacBheatha.
