When the paper is in contact with water, the energy from the open flame that would usually start the combustion reaction is transferred to the water before the paper can ignite. This raises the temperature of the water and it will eventually reach boiling point. Now when all the water has evaporated, then the cup burns because there is nowhere left for the heat to be transferred to.

By putting the water in a pan/pot and then hold the pan/pot over the fire until hot :)

You can pop a boil but it is not recommended due the risk of further infection. You can make the boil come to a head by laying a washcloth rinsed in hot water over it. The will help the infection rise to the surface. As this happens the boil will open up and begin to drain.

The water will turn into water vapor which is gaseous H2O. If the vessel is not strong enough to withstand the increased pressure applied in all directions the vessel will open or break if possible. Otherwise the pressure will just increase based on how much steam is created.
