Humidity - water vapor mixed with air - is drawn to the coolest surfaces, like windows. Cool air can't hold as much moisture as warm air so windows often collect this moisture and make it visible.
The water vapor comes from your lungs, specifically from the moisture in the air you breathe out. When warm air from your breath comes into contact with the cold window pane, it cools rapidly, causing the moisture in it to condense into tiny water droplets on the glass.
When you breathe on a cold window, the warm air from your breath hits the cold surface of the window and cools down. As the warm air cools, its ability to hold moisture decreases, causing the water vapor in your breath to condense and form tiny water droplets on the window, creating the steamed-up effect.
Condensation can form on double pane windows when there is a problem with the window's seal. If the seal is damaged or broken, moisture can enter between the panes and then condense when the temperature changes. This indicates that the window's insulation may not be as effective, and the window may need to be repaired or replaced.

It is the moisture leaking through the seal of the window. The moisture collects between the window panes and makes the glass fog up. I do not know how i can fix it. I think you just have to buy an all new window.
To prevent moisture from accumulating inside your window, you can improve ventilation by opening windows regularly, using a dehumidifier, sealing any gaps or cracks around the window, and using insulating window coverings.

Condensation, the cause of which is usually related to humidity, on a window can be a sign of over humidification, no dehumidifying, low R factor windows and even drafting. The question is a little vague. If it were a lot in the winter I would see if my humidifier needed to be turned down.
