It depends on how you personally define "boy" and "girl" - some people get confused because they struggle with other people's expectations and definitions and question their own gender because they don't conform to what others expect.

It is quite common for boys who do not conform to their local culture's defined expectations for boys to be accused of being "too feminine". In that case the issue is not really whether they are a girl but rather that others are trying to pigeonhole them. They are still very much a boy - just not what someone else defines as a boy.

Likewise many girls get labeled (derogatorily) as "tomboys" or male-wannabes just because their interests have been defined by others as being male. They are still very much female - just not what others expect them to be.
