Sometimes they really mean it and sometimes they are just saying it to make you feel better (its unfortunate but they do).
Often enough, although poorly worded, it is what they feel. Othertimes they may just be saying something to be mean cause they feel hurt. If someone did say something bad when drunk, I'd recommend discussing when they are sober to get a better idea of what they mean, cause they will be better able to communicate it.
