Currently (as of March 3, 2013), it costs $2.75 for a single ride ticket, and $2.50 per ride for a pay-per-ride Metrocard.
There is now a $1 fee for purchasing a new pay-per-ride (plastic) Metrocard, so always refill cards whenever possible. You are exempted from the $1 fee if your Metrocard is expiring or damaged.
The same Metrocards are used for the city buses. Pay-per-ride Metrocards give you a free transfer between the subway and bus, or between buses.
You can also purchase Unlimited Metrocards, which give you an unlimited number of rides for a flat fee. There is a 30-day Unlimited card ($112), and a 7-day Unlimited card ($30).
Keep in mind that Unlimited cards will not work again for 5 minutes after they are swiped. This is to deter people from using one card for multiple people. You can still use the Unlimited cards for multiple people, but you must be prepared to wait 5 minutes after each swipe.
