Mormons believe that monogamy is ordained of God, yes, but they do not believe this has always been an unconditional requirement. There were many great men in The Bible who were not monogamists, and God even approved of their having multiple wives. Mormons believe that there have been certain times throughout history where certain people were either commanded or allowed to take additional wives - monogamy is the rule unless commanded otherwise by God.

Yes. In fact, members of the Church of Jesus Christ of Latter-day Saints (the "Mormon" church) practice only monogamy. Bigamous and polygamous marriages were accepted in the church from the early 1840's until 1890. However, in October 1890, polygamous marriages were banned, and from that time on, the church accepted only monogamous marriages. This ban was reiterated in 1910. Today, anyone found in a polygamous marriage is excommunicated from the Church.

There were some who disagreed with this ban who split with the Church of Jesus Christ of Latter-day Saints to form their own groups. Today, there are several groups, such as the Fundamentalist LDS and the Apostolic United Brethren, who accept and practice bigamous and polygamous marriages. While these groups are not a part of the more well-known "Mormon" church, these groups still fit in the "Mormonism" category, since they have the same background and similar beliefs. However, these groups also accept monogamous marriages and do not require all their members to participate in polygamy.
