It is not based upon anything else. Though people always fall short and fall into sin ( no one is perfect ) yet God has great Mercy - Grace - for those who try their best. Ultimately, it is up to God, alone, to say if you are successful or a failure. He is the Only Judge. 5) All people will be raised to life again on the Day of Judgment. Then, every individual will either be in Paradise, or Hell. Paradise is permanent. For some, Hell will be permanent; but for others, God will remove them from Hell - after their being punished - and place them in Paradise. But those are believers who sinned.
