Mohammad wrote a kind of 'bible': the Koran. Obviously inspired by the Roman Christian 'bible'
Muhammadan doctrine has five aspects which confusingly became known as the five pillars of all of Islam:
Kalima ; To agree that God is one and Mohammad is His Messenger ('Kalima' obviously is Mohammedan, and is absolutism)
Salat ; To say five daily prayers at their appointed times (folklore).
Fasting ; To observe fast during the month of Ramadhan (folklore).
Zakat ; To contribute a certain percentage of wealth for charity (before Mohammed 'Zakat' was much stricter, aimed at ONE wealthy family))
Haj ; To go for pilgrimage to Mecca at least once in one's life-time if physically and financially possible (folklore)
Mohammad's followers added 'Kalima' to Islam and changed 'Zakat'
A 'Mohammedan' sees 'Kalima' as law of nature
There are very little about when Muhammadan exited. It has been told that Muhammadan did start centuries ago.

It's hard to tell in what sense you mean, but Islam is a "Muhammadan" religion, as Muhammadan/Mohammedan means, as a noun, a follower of the prophet Mohammed, or as an ajective, relating to the prophet Mohammed or the religion he founded. Islam is also one of the three Abrahamic religions, that is, one of the monotheistic religions that has Abraham as an important figure within the religion. The other two are Christianity and Judaism.

No other names. Some call a Muslim as Muhammadan (after prophet Muhammad as Christians named after the Christ) which is wrong because also prophet Muhammad (PBUH) is Muslim.
That would be the shahada. La Ilaha Il Allah. Muhammadan Rasoul Allah. That translates as: There is no god but (one) god. (And) Muhammad is his final messenger.
anna ان is for insist and means "that" for example اشهد ان محمد رسول الله ashhadu anna Muhammadan rasul al-allah means: I wittiness "that" Muhammad is messenger of Allah

After performing wudhu, it is recommended to recite the dua: "Ashhadu an la ilaha illallah wahdahu la sharika lahu wa ashhadu anna Muhammadan abduhu wa rasuluhu."
The shahada is the same whenever you say it: "Ash-shadu an la illah il-Allah, wa ash-shadu Anna Muhammadan rasul-Allah." (I testify that there is no god other than Allah, and I testify the Muhammad is a prophet of Allah.)
The proper dua for making wudu is: "Ash-hadu an la ilaha illallah, wa ash-hadu anna Muhammadan 'abduhu wa rasuluh." This translates to "I bear witness that there is no god but Allah, and I bear witness that Muhammad is His servant and messenger."
