Hajj is the 5th and last pillar of Islam. It becomes obligatory for Muslims (males and females) to perform when becoming adult and when physically and financially capable to perform it. A Muslim woman has one more condition: to have a custodian to accompany her like her husband or whoever cannot marry her such as her father, brother, or an uncle.

When a Muslim goes on Hajj, he first must have the intention to do so, and ask the others to forgive him if he did any wrong to them.

When a Muslim reaches Al-Miqat "A place where a pilgrim should wash, take off his clothes and wear two white blankets on', 4 Miqats are in the 4 directions of Makkah. He will wash, take off his clothes, take a shower and put on two white garments. Then he will utter the Niyyah "Niyyah in Islam is silent except in Hajj and Omrah".

When he reaches the holy Masjid of Ka'aba, he will circulate seven times around Ka'aba starting from the black stone at his left. He will touch it if close and able to, or points out to it by hand and says: Bismillah Wallahu Akbar" meaning in the name of Allah the greatest. When he reaches the black stone again it is considered one circle. Upon completing seven circles, he will run between Al-Safa and Al-Marwa mountains following Hajar, Abraham's wife, who did this while looking for people and water for her son after Abraham left her in Makkah.

On the 8th of Dul Hijjah, the Pligrims will go to Arafat and stay there until sunset. After that they go to Muzdalifah where they spend the night and collect 7 stones to throw next day "Eid Al-Adha".

After throwing the stones, a Muslim cuts his hair, usually using a knife, slaughters his goat if he has any and then goes to Makkah for perform Ifadah and farewell "7 Circles" around the Ka'aba and leaves Makkah back home.

This is a summary of what a Muslim does on his Hajj trip.

Why does other Muslims do not wear underwear?

That is not true. There is nothing that may prevent Muslims from doing so. However, Muslims never wear underwear on Hajj as that is one of the rules/ traditions. It is the same in Pakistan and many hot countries where it is more healthy not to wear, and I prefer not to. But no religious reason except for Hajj.
