Because some religions are told to kill everyone who does not conform to their faith... the people who ignore this tidbit of info are thus deemed "moderates" while the ones who faithfully follow their teachings are deemed "extremists". Other religions involve teachings of loving your neighbour as yourself, and turning the other cheek. People who follow this are not generally noticed by the mainstream, while people who do not follow this are quickly pointed out for their often-verbal association with a certain faith.

The most fundamental reason is because there is no possibility of a common, universally accepted religious truth. Out of that come the fundamental conclusions of many systems of faith and religion. Many of these conclusions have to do with a vengeful deity bringing down destruction on those who do not believe the 'true religion'. These go hand in hand with promises of glory and reward in heaven to those who are vigilant and zealous in the struggle to gain believers and destroy everyone else. But since different groups have sometimes vastly different beliefs of what constitutes religion and faith, what distinguishes true believers from infidels and how the deity deals with sin and forgiveness, clashes are inevitable. Not everyone can be right. Many end up concluding that anyone who disagrees with me must go. This might be justified as ok because they are enemies of the deity.
The above describes what happens in the most extreme cases of conflict. Even in gentler times, the differences are there and they constantly inform and remind members of a given religion that while we might tolerate those who disagree with us, we know where 'we' are ending up, and where 'they' are ending up. This is very much like the nicknames and epithets employed by governments in attempts to de-humanize the general populations of enemy states.

Whether a religion is genuinely of God or not, all religions are handed down to men. Sometimes just once. Sometimes many times, over a long period of time.
Now; whilst them men live, they normally have a pretty good idea of what it is they believe. If we could go back in time and ask the "founder" about a current problem, then that ought to be the best person to ask.
Better still though, is when a religion is progressive. Then, you can have revivals and men can actually increase in light. But not within a given group or denomination. The tendency is for man to organize God clean out of the program over time. Replacing spontaneity, fresh manna, and inspiration with formula, ritual and dead works.
Unless you have some system whereby God can show His approval of a particular belief with regards to something, then you're left with speculation, argument, debate, logical reasoning (with and without flaws), etc...
But even where that system DOES exist, there is nothing to say that there shall be any person at any given time who is spiritual enough and enlightened enough to receive both the correct teachings and specific answers to questions needing answering.
