Yes they do. Just as much as it would hurt for any normal guy. Though years of hormone therapy shrink the testicles to a diminutive size, they still retain all the nerve endings, connections and physical structures one finds in a normal pair of testicles.

This also depends on the kind of shemale you're talking about. If it's a post-op, she's had her testicles removed and the pubic area converted into something that very closely resembles natural female genitalia. In that case, there are no testicles to hurt and she will not feel any pain other than what a natural born woman would if she received a blow there.

Please also note that, real shemales have very tiny testicles and most times, the chances of connecting a blow with them is dismal. However, the fact remains that they're just as sensitive as normal testicles.
