You need to look for signs of the other sex.

like, if a woman has:

more pubic hair around vagina and under arms

larger hands and feet than women

broader shoulders and chest

larger skull and bone structure

greater muscle mass

a more prominent Adam's apple and deeper voice

A longer Shinbone

then she could have been a man.

So, if a man does not have those examples, he might (not is) have been a woman.

it all depends on how large the surgery was.

Answer:

Sometimes you can't tell. If the person had the surgery at 18 or 19 years old and they had been on anti-androgens (holding back puberty) then they could be missing all the classical secondary sexual characteristics:

no thick brow, smaller hands and feet, very little hair, smaller bone structure, very small or no Adam's apple, less muscle mass.

Some of these things can be changed by HRT, Facial Feminizing surgery, and Adam's apple shaving.

Also note: lot's of women have hair, including facial hair, hairy bodies, bigger hands, thicker brows etc. Just because you see or don't see things doesn't mean they are what you think. There are some guys born that need no surgery and are prettier than most women. There are some women born more masculine than most males (think about the joke about East German women...)
