I know a straight man who is frequently called Ma'am on the phone. You may be talking about some gay men speaking in an effeminate manner. This is learned behaviour and most gay men do not speak that way.

You can't, really. Some guys who act "feminine" are straight; some guys who could rip your arm off are gay. But if you show you're comfortable with gay people by doing things like joining a gay-straight alliance, or objecting when people make "fag" jokes, gay guys will be more likely to trust you.
