In my opinion the song is about a paranoid schizophrenic (at least someone in a mental institution). The knitting terms (pick up every stitch & running in a ditch) emphasize the pass-time in the institution. Looking out of a window on a world they are separated from provides lots to see. Looking into the window (reflection) reveals different people to be, hence the schizo. Looking over their shoulder and seeing a stranger doing the same thing.... maybe they are looking into a mirror further emphasizing their detachment from themself. Well, would it not make sense to a person who trusts no one or is not sure anyone is who they say they are (not the people who take care of them or the people on the streets or even themselves) that it MUST be Halloween. The lyrics for me carry a message of Donovan's outlook on society at the time from cultural conformity & disillusionment to the abuse of vulnerable persons and perhaps a connection has been made between Social & Mental Disease.
