She doesn't have one. The use of middle names in Japan is not very common, and Kagome is one of millions of modern Japanese people without a middle name.

Yes, but the anime skips around a lot and glosses over a lot of things talked about in-depth in the manga.

Also, Season 4 of "Inuyasha" is almost completely fillers. The anime had caught up to the manga already published, and, to avoid halting production altogether, created completely random episodes that didn't serve a large plot purpose (so that they wouldn't interrupt the real story). Such as the character Ayame, created in the anime to be Koga's bride-to-be, even though she didn't exist in the manga.

The manga, as with any book to a movie, is a lot better than the anime, but both are excellent!!

Yes: they are played by the same voice actor. In the Japanese, it is Kappei Yamaguchi, while in English, he is voiced by Richard Cox, who provided the voice of guy-type Ranma.

It's just called 'Neji's Theme'. It's not on any of the official Naruto Soundtracks so you normally have to find it seperatly.
