You don't craft collars for anything. You're probably thinking how wolves have them. People tame them by right clicking them with bones. Then they automatically get a collar.

First, you must install the Aether mod (which is huge, by the way.) Then, in order to make an aether portal, you must have at least 14 glowstone blocks. Build it like this:

When you're done, pour water in the interior of the glowstone. This will make a portal to Aether.

It wasn't really realeased, there was a so called "leak" by Jeb so you download it, repalce you're .jar and play 1.9 but it still has many many bugs so it wasn't officially realeased.

I don't believe that Herobrine is in vanilla Minecraft. There are videos on YouTube that used the Single Player Commands mod to spawn the discontinued 'monster' (human) mob with the texture modified to look like the legendary Herobrine.

The SPC command is 'spawn monster'.

There are 'Herobrine' mods, but I have not looked into those.

You'll have to find a stronghold. They're randomly in blocks.

Edit for updated game versions: They can now be randomly found in Extreme Hills biomes as well.
