Pap Smear

The Papanicolaou test (also called Pap smear, Pap test, cervical smear, or smear test) was invented by Georgios Nikolaou Papanikolaou. He was a Greek pioneer in cytopathology and early cancer detection. The PAP test is a screening test to detect possible pre-cancerous and cancerous cells of the endo-cervical canal. A plastic a speculum is used to open the vaginal canal, and an applicator is used to very gently collect cells from the end of the cervix. Special stains are added to the specimen, which is sent to a lab for processing. Cervical cancers are typically caused by sexually transmitted human papillomaviruses (HPV) which most females contract soon after first becoming sexually active. The American Congress of Obstetricians and Gynecologists (ACOG) and other experts recommend starting screening at age 21 (or between ages 20 to 60, and definitely while sexually active. The test is not painful, though it can be temporarily uncomfortable.

You should wear clothes that are easy to take off. Like a skirt with sandals or jeans and flip flops. The nurse will give you a hospital-like gown to put on. You will be asked to remove your underwear and pants/skirt and lie on a table. She will look and feel of your external vaginal area (such as the labia) and feel of your uterus on the outside. She will then insert a speculum (metal object that will open your vaginal canal)into your vagina (usually after a lubrication is applied and look at your cervix and take a swab of your cervix. It is usually not painful, just uncomfortable.

no. a pap smear will test for general cell abnormalities of the cervix. the doctor maybe able to better determine via visual inspection during the pap smear if further testing for anything visually abnormal may be needed (std's, etc). Fertility usually involves a test of a females ovarian reserve (usually measured on days 2-4 of a menstrual cycle, the norm being day 3) which can be done with a urine test (at home) or blood test. this mesure the quantity and quality of the eggs you have left. there are of course other tests to determine the health and vigor of the ovaries, fallopian tubes, etc. But a pap smear cannot be used to determine fertility.

I too have very painful pap smears which seems to be because I can't relax. I have not had sexual intercoursre per say but have experimented with sexual toys at times. The last time I went through one, it was extremely painful and I had the doctor at the end of the table barking orders at me in very terse and irritated tone, telling me to relax. I think there are just doctors out there male and female, who simply don't want to acknowledge that the relaxing is not just something that woman can always get over. I've heard some people tell me that I should take Valium but I don't know how to get it. It's ridiculous and really pitiful that even female gynecologist arn't willing to address this problem with some sort of drug or some sort of topical numbing agent or something made readily available to women during these exams. As it is now, I haven't had one in 6 years because the last one was so humiliating with a doctor practically screaming at me at the end of the table to relax. The entire state of women's health issues is in disarray and this is just one very important example of this!

I hear a lot of people say, "Oh, a pap smear doesn't hurt... it's just uncomfortable." Well, mine hurt today and have always hurt. It is both uncomfortable and moderately painful. Maybe a pain level comparable to someone yanking your hair. It hurts when it happens and then there's a smarting soreness that slowly fades. Not like you can't bear it, but why volunteer?

Theories as to why it hurts us two and nobody else: 1) we're the only people with nerve endings on our cervix 2) Speculums are made of velvet at every other doctor's office and our doctors use iron ones with spikes [note: I find this the "uncomfortable" part, but I do not see how someone is saying that is not also slightly painful.]3) It could be that you have one or more small growths called "fibroids" growing in an area that is being crushed by the speculum or the doctor's hand. Fibroids are like skin tags that grow on the inside of the uterus. 4) You are tensed up and because your're not relaxed it hurts more. [I don't buy this suggestion AT ALL] I am a fearless person. I tolerate pain well and am not squeamish.

Perhaps worst of all, every single time, [except for today] the doctor [male or female, I have never gone back to the same one] has made what I consider to be inappropriate comments. So, I avoid pap smears as much as possible. Of course, the medical profession does not recommend this.

A pap smear may be uncomfortable but should never be painful. Pain could be numerous reasons; - a pulled pubic hair, - not parting the labia enough or parting it too far before inserting the speculum, - if the cervix is quite close to the entrance of the vagina, the speculum may be pushing on it when opening, - the instruments used can cause discomfort if a woman is quite sensitive. The most common reason, would be that a Dr/Nurse is not watching the cervix as they remove the speculum. Sometimes the cervix can cause "suction" where it is sitting quite snuggly in the speculum. When the clinician removed the speculum without watching they may be pulling on the cervix/uterus, which can cause quite a lot of pain/discomfort. Women who have had this can have a dragging sensation and be quite crampy for a day after their pap smear. This shouldn't be happening. A clinician should be watching the speculum, and if "stuck" using techniques to remove the speculum without causing any discomfort. Another reason for pain, could be an STI and the swabs used for a pap smear can also be tested for Chlamydia and Gonorrhoea. In general, a screening pap smear performed every 2 years (In Australia), should not be painful and the only discomfort should be the psychological barriers from the woman. These can be reduced if the clinician is awesome! :)

If the pap smear was done for other reasons (e.g. bleeding, fibroids), then pap smears may be discontinued. Talk with your health care provider for advice specific to your situation. Annual pelvic exams are recommended in any case, with or without the pap smear.

A speculum is a tool for keeping the vagina open so that a medical person can see the inside of the vagina and cervix. It is either made of plastic or metal. Plastic ones are throwm away after use, but the metal ones are washed, sterilised and reused.

There are several different kinds of vaginal speculums, but the one in most common use is called a Cuscoe's Speculum. There are also Sims, Fergusons, Auvards and a few more, but the Cuscoe's - or 'duckbill' as it is sometimes called - is the one you are most likely to come into contact with.
