well, there is the first stagate : "Stargate sg-1" then "Stargate Atlantis" follows it, and the last stagate show is "stargate Universe".

In stargate SG-1 What episodes does Samantha Carter get hurt or injured in?

Here are the episodes I can remember where she gets hurt or injured:

-The Broca Divide: She is stabbed by a cell mate as her and almost everyone else on base de-evolves.

-The Nox: Actually, in this episode she is killed along with the rest of SG-1 by staff blast, but the Nox bring them back to life.

-Solitudes: Sam and Jack are both thrown through the Stargate in Antartica. I am not sure her exact injuries, but I'm sure she was bruised.

-There But For The Grace of God: In this episode, only an alternate reality Sam is killed when she blows herself up along with a bunch of Jaffa.

-In the Line of Duty: Sam is taken host by a Tok'ra symbiote.

-Prisoners: Sam gets in a fight with some criminals on a prison planet.

-Jolinar's Memories (part 1)/ The Devil You Know (part 2): She experiences, through her memory, of Jolinar being tortured. She is tortured when they are put in a literal hell.

-Urgo: She has a small device implanted in her brain (along with the rest of SG-1) that allows her to see and hear an alien technology (looks like a guy) known as Urgo. Urgo accidently makes Sam spill hot coffee on herself and she gets a burn.

-2010: Alternate timeline Sam is killed while trying to get a note through the gate.

-Entity: I think she burns her hand, but I am sure that a computer program takes over her body and her conciousness is downloaded into a computer.

-Ascension: She passes out when an ascended ancient tried to "connect" with her and she wasn't prepared.

-Grace: Sam takes a bump to the head on the Prometheus. A bad bump.

-Morpheus: SG-1 and the other team with them is affected by a "bug" that makes you sleepy and when you do fall asleep, you never wake up.

There are quite a few more episodes where Samantha Carter gets hurt or injured, but I am not sure which.

She is essentially inactive after she gated to outer space from Atlantis.She continues to merely float in space. According to producer , Joseph Mallozzi the character will not be seen again in the Stargate Atlantis film entitled, Stargate Extinction.

No you cannot go through an incoming wormhole. In the Stargate series a wormhole is a one way ride.The one way nature of the Stargates helps to defend the gate from unwanted incursions. Stargates are also only capable of sustaining and artificial wormhole for 38 minutes. It's possible to keep it active for a longer period, but it would take immense amounts of energy.

What happened to Dr Weir in Stargate Atlantis?

When Elizabeth Weir was seriously wounded by the Asuras (a form of replicators invented by the Ancients to combat the Wraith), Rodney McKay injected her with deactivated nanites which will repair her damage, although in the end she will be no longer fully human, but part replicator. She sacrificed herself when Sheppard's team was on a mission to steal Zero Point Modules from the Asuras. A replicator version of Jennifer Keller told the team that later after they abandoned Elizabeth, she was killed by Oberoth. But apparently, this was not the case.

Elizabeth Weir teamed up with a couple of replicators that wanted to find a way to ascend like the ancients. She and the other replicators did ascend, but only 'digitally'. In the episode: Ghost in the Machine, Elizabeth Weir came back to Atlantis and took control of Atlantis's computer systems and made bodies for her and the other replicators, using Rodney's FRAN templates. Later in the episode, a replicator named Koracean tries to escape, finally understanding that she and the other replicators are only danger to Atlantis, Elizabeth managed to fool the other replicators (with the help of the team) into a Stargate to a space gate, making the replicators including herself neutralized.
