You cannot flag a question. You can only flag answers to questions. To do so, you must be a registered user, and if you look under 'Question Tools' in your blue navigation bar, it should say 'Flag for Improvement'.
There is no way to flag questions for improvement currently on Answers.com. A signed-in member can flag an answered question by using the "Flag" option on the Question tools menu. You can "Report abuse" on questions that don't have answers but you shouldn't use that tool just for flagging something as not good enough.

Nobody can flag an unanswered question, and only members can flag questions that do have answers. We do this by selecting the option "Flag" from our command list, supplied on the side of the window.
Answers (and many Questions) that contain the word APEX are questions used in an online school called Apex, Inc. Students from Apex classes try to get homework answers. Please feel free to flag or report any questions/answers that appear to relate to the Apex online homework questions.

However, that is how it is designed.Now, if you mean wrong or inappropriate answers to your questions, that is the work of trolls and vandals. They simply post whatever to feel important, see their own handiwork, annoy others, impress others who are like them, or whatever. Really, just flag or report those when you find them.
Indeed you can. If you want to edit or remove flags, go to your Site tools, and there should be something marked "Flagged questions". Click that, and you will be directed to all the flag questions, where you can remove, edit, or add flags onto questions and/or answers.
Not all answers on WikiAnswers are useless. Usually unhelpful answers are posted by WikiAnswers users who don't know what they are doing, and post answers to questions even if they don't really know the answer. If you find an answer on WikiAnswers that is unhelpful, you can remove it or flag it for improvement using the tools on the blue bar on your left.
WikiAnswers bots do not randomly target and flag questions; they use an algorithm to determine answers that should be flagged based on a list of inappropriate words. As with humans, bots do make mistakes, so if you come across an unwarranted flag, please remove it.
