To the extent that the US flag represents all the sovereign territory of the United States, it includes both Hawaii and Puerto Rico. However, the stars on the US Flag represent the states of the union. Hawaii is a state and so is represented on that part of the flag. Puerto Rico is a Commonwealth in Free Association with the United States, not a state, so it is not represented among the stars on the US flag.

Maryland was admitted to the Union April 28, 1788, the seventh State to be admitted. Although the U.S. flag has progressed through several different designs, all with 15 or 13 (13 after 1818) red and white stripes with a blue union--each has had a differing number of stars in different configurations. As Maryland was among the original 13 States, one of the 13 stars on that flag represented Maryland, and Maryland has been represented with a star on each succeeding design of the U.S. flag since that time.

There was no set pattern for stars on the US flag and the individual who proposed the flag, Francis Hopkinson, did not set them in a circle but in the pattern of the crosses on the flag of Great Britain. Thus they were scattered.
