Its pretty much the same as a normal psn account. First is you go to Where you'll see the PlayStation network sign, click it. After that press some other stuff and write your address and all that then make an acount name (NOTE* You can not change your online ID so make sure your ok with with name u have because u can not change it). After you press ok CONGRAGULATION U HAVE A PSN ACCOUNT.
You can not make a sub account into a master account because it is already under a Playstation master account. If you create a new account under a different name you lose your trophies and game saves

A 'Master' account only means something in the context of having a sub account associated with it. By that I mean, any normal account becomes a master account if you create a sub account on it. So you don't technically create master accounts.
