Firstly you need to mount the .iso image. To do this right click on the file and either 'open with' or 'mount' with poweriso. If those options aren't on the right click menu, open up power iso and browse for the .iso image file. After it has been mounted you treat it like a normal disk. So go onto my computer and find the fallout 3 'disc' and inside it should be the setup.exe, run it, follow the instructions and enjoy!
Depedning on how you obtained the .iso image you may have to have the .iso image mounted when you run the game as it may have a disc check system. Hope this helped!
