Until 1993, there has no been no known break-out from Azkaban when Sirius Black managed to break out. In 1996 and 1997 there were mass breakouts of Lord Voldemort's death eaters. The name appears in the title of the third book, Harry Potter and the Prisoner of Azkaban.
