No one really knows. 343 Industries has said that it is not the last we have seen of the Master Chief and Cortona. That could mean another game, but it could also mean the story's continuation from another medium. So as of right now, no one ones.
Yes. There is a whole new trilogy called The Reclaimer Trilogy. It consists of Halo 4,5,6. There probably will be some spin-off games as well. Not much about the trilogy is revealed at the moment. Halo 4 will probably be out Nov 2012. There is also a book trilogy called Glasslands that you can check out as well. And also check out the short story/motion comic called The Return. And I'm sure there will be much more in the future.
As far as I know, REACH is the final halo. They said it on TV.
Correction: There will be at least one other Halo game, for now titled Halo 4. It is set to be released on 6 November 2012, and is supposed to be the first part of a new trilogy, the "Reclaimer Trilogy". Unless 343 Industries, Microsoft's in-house development studio for everything Halo, messes up this game (which is rather unlikely), we are going to see at least two more games and maybe - if we're lucky - also spin-offs like Halo3:ODST.
I have Halo 4 and me and my friend has finished it. At the end, it wasnt so clear if the series is over. But the clip at the end very well seemed as it was over.

yes, it is set at the battle of Reach which was where the Pillar of Autumn (the ship in Halo 1) was right before it jumped into slip space and found the original Halo. In short, it is set before Halo 1 hence it is before Halo 3
