The question can be interpreted two different ways.

1) What event in 1948 caused continual tensions in the Middle East? -- The answer to this is the Creation of the State of Israel and the resulting Arab-Israeli War of 1948-9 and aftermath. Israel is the source of instability in the Middle East by continuing occupying the Palestinian lands and preventing them from living a dignifying life.

2) Since 1948, what events have contributed to Middle East tensions? -- There are numerous Middle East events since 1948 that have caused or added tensions. There have been numerous Arab-Israeli Wars and Israeli-Palestinian Engagements. There have also been numerous coups d'état (like those in Turkey, Algeria, and Libya). There have also been revolutions and removals of authority (such as the Islamic Revolution in Iran and the End of the Monarchies in Iraq and Egypt). There have also been huge power grabs by existing authorities such as the Emergency Powers Law in Syria and expansion of authority by Hosni Mubarak. Basically, everything in the Middle East causes tension.
