Just like with the wild mustangs and ponies of Chincoteague Island, spotting or patchy colouration evolved through genetic mutations of the solid coat-colour allele, allowing cattle, even those in the wild, to obtain patches over their body instead of being a solid colouration.
It is thought by some that it evolved through artificial selection or selective breeding so that people could get cows of this colouration for aesthetic purposes, however documented evidence of feral cattle of the wilds of the United Kingdom dating back to the 17th century stated that cattle, such as those that were created into the Aberdeen Angus breed, were patched and spotted well before people "turned" these cattle into black, solid-coloured and polled cattle we know of the Angus breed (and other breeds)today.
Depends on the breed: from none to hundreds.
This question is irrelevant because not all cows have spots and those that have spots are of a particular breed like Holstein, for instance. The "average" cow cannot ever be expected to have spots--quite frankly, the average cow may be more solid-coloured than spotted!!
It's a white box with large, irregular black spots on it.

no horns, not charging, sounds like cow, size, has spots, see udders
Only if that cow originally had spots before it was shaved.
Some cows have black spot. some have brown spots. Some times it could be a black cow with white spots.

Bob the talking cow
Black Holsteins are white with black spots, red Holsteins are white with red spots.
Many people have thought they have seen different images in the spots on a cow. It is possible to imagine a world map on the side of a cow.
Giraffes, you idiot... Or a golden cow.
The oh-so popular Holstein cow.

No, there is not a breed named a painted cow. Holsteins are the most common breed that may be mistaken for similarities of the the Paint horse breed. Holsteins have large black and white spots.
