Ions are atoms that have either gained or lost electrons. Ions are normally formed by chemical processes. For example the rusting of Iron yields various types of Iron Oxides. Iron metal and oxygen gas both become ions. So when an atom becomes an ion through chemistry it always has a counter-ion (one positive and one negative).
Some ions can vary in their charge depending on the situation, for example Iron often has a 2+ charge but can also easily have a 3+ charge. However, this is not formally considered "an ion of ion."
